Oral Surgery

Effective January 1, 2023, all out-of-network oral surgery claims will be covered at in-network benefit levels.  Out-of-network oral surgery claims will be covered by the Plan subject to the in-network annual Deductible, in-network Coinsurance, and in-network annual Out-of-Pocket Maximum (including Deductible).
